Patient Rights and Responsibilltes

Rights as a Patient include:

  • To confidentiality of your medical information.
  • To understand your diagnosis, treatment, and prognosis.
  • To have this information in a language you can understand.
  • To have an interpreter available if needed.
  • To be scheduled within 24 hours for emergencies, and a week for non emergencies. To  wait less then 30 minuets for your appointment.
  • To know the reasons, costs, and risks for testing procedures and treatments
  • To request a consultation with another  provider
  • To accept the risks or refuse a treatment of procedure
  • To have access to your medical records
  • To have modifications available for disabilities
  • To have no discrimination for race, color, national origin, disability, age, sex, religion, creed, or method of payment

Your Responsibilities include:

  • To respect other patient’s privacy
  • To learn about your medical condition
  • To communicate honestly and clearly with your practitioner
  • To come in at your appointed time, or call when you need to make schedule changes
  • To refrain from heavy perfumes and smoking prior to entering the clinic  (for those who are chemically sensitive)
  • To take the necessary steps to maintain good health
  • To notify us if you do not understand or need more explanation concerning your diagnosis, treatment, or progress
  • To promote the good health of our society and planet
