Rights as a Patient include:
- To confidentiality of your medical information.
- To understand your diagnosis, treatment, and prognosis.
- To have this information in a language you can understand.
- To have an interpreter available if needed.
- To be scheduled within 24 hours for emergencies, and a week for non emergencies. To wait less then 30 minuets for your appointment.
- To know the reasons, costs, and risks for testing procedures and treatments
- To request a consultation with another provider
- To accept the risks or refuse a treatment of procedure
- To have access to your medical records
- To have modifications available for disabilities
- To have no discrimination for race, color, national origin, disability, age, sex, religion, creed, or method of payment
Your Responsibilities include:
- To respect other patient’s privacy
- To learn about your medical condition
- To communicate honestly and clearly with your practitioner
- To come in at your appointed time, or call when you need to make schedule changes
- To refrain from heavy perfumes and smoking prior to entering the clinic (for those who are chemically sensitive)
- To take the necessary steps to maintain good health
- To notify us if you do not understand or need more explanation concerning your diagnosis, treatment, or progress
- To promote the good health of our society and planet
